At The Wellness Plan Medical Centers, we ask that you come to appointments prepared with proper paperwork (including test results from other medical centers). It also helps if you are prepared to answer medical questions about existing conditions. When visiting a TWP location, please bring a valid driver’s license, proof of insurance (if applicable), prescriptions, and any questions you want to ask your healthcare provider. If your appointment is a follow-up appointment, it is helpful to bring prior test result records.
If you are a new patient, please arrive 30 minutes before your appointment to fill out paperwork. If you are a returning TWP patient, please arrive 15 minutes before.
