About Us
The Wellness Plan Medical Centers (TWP) is comprised of five medical centers located in metro Detroit. We are committed to delivering quality primary care and health services to uninsured and underinsured residents, regardless of ability to pay. We are recognized by The National Committee for Quality Assurance (NCQA) as a PCMH. The NCQA seal is a widely recognized symbol of quality.
A PCMH is a model of health care that makes patients the number one priority. PCMHs strive to build stronger relationships between medical professionals and their patients. With this idea in mind, it will result in overall better patient experience and staff satisfaction. With these factors in mind, PCMHs also work to reduce health care costs.
Our staff is dedicated to making sure our patients are comfortable and are given all information needed in order to be well and stay well.
Our goal is to provide high-quality healthcare while treating patients with dignity and respect.
Our service providers work to form personal relationship with our patients that serve as the foundation for a Patient Centered Medical Home (PCMH).

At TWP, our vision is to provide optimal, trustworthy health services to the metro Detroit community, primarily to those who are underinsured or uninsured. TWP is a values-based medical center and a trusted Patient Centered Medical Home (PCMH). As a PCMH, our healthcare services are coordinated through a medical professional to offer the necessary care for all of our patients, in a friendly, caring manner. In addition, we are determined to deliver quality care as we provide timely services to the metro Detroit community.
Federally Qualified Health Center (FQHC)
TWP is a non-profit 501(c)(3) Federally Qualified Health Center (FQHC) and is a Federal Tort Claims Act (FTCA) deemed facility, which means it receives funding and other liability coverage benefits from the federal government. FQHCs are important resources in many low-income communities, especially in rural and urban areas. An FQHC is an outpatient clinic that qualifies for reimbursement systems under Medicare and Medicaid.
FQHC’s must meet a stringent set of requirements, including providing discounted care on a sliding fee scale based on family size and household income and operating under a governing board that includes patients, according to the Health Resources and Services Administration (HRSA).
As an FQHC, TWP offers services to all, regardless of a person’s ability to pay. We are also a nonprofit organization that is community-based, with a majority of the board of directors as patients. The benefits of being a FQHC is the ability to be funded to offer trusted, quality care to those who would otherwise not have access.
