The Wellness Plan Medical Centers is a patient-centered medical home (PCMH). As a PCMH, TWP is an active participant in working to improve healthcare in America, by transforming how primary care is given. This model of organization allows medical centers to provide patient-centered care. We are happy to coordinate all care for you, through your primary care physician, and organizing specific services based on patient needs. As a PCMH, we are committed to offering accessible and safe care for all.
PCMH Approach
According to the American Academy of Family Physicians, a PCMH is a medical approach based on the following principals:
- Each patient has an ongoing relationship with a physician
- The personal physician leads a team of medical professionals in various areas
- The personal physician is responsible for the entirety of the needs of the patient, and arranged qualified professionals as needed
- All health services are accounted for across the whole health system
- Ensuring all services are up to health standards and often exceed baseline requirements
- Accessible care is provided through convenient hours, open scheduling and timely communication between patients and medical staff
- A payment structure that meets patient needs and is clearly communicated by the health provider
Provider and Patient Responsibilities
Patient rights are basic rules of conduct between patients and medical professionals. A patient is anyone who has sought treatment at a healthcare centers’ locations, including clinics, pharmacy, and urgent cares. Patient rights include:
