Community

The Wellness Plan Medical Centers strives to stay connected with its patients and the community with a variety of events and activities. From family literacy programs to informative webinars to Family Fun Days, we do what we can to make healthcare both fun and informative. Our community outreach team offers programming that covers health education, including health promotion and disease prevention. Other educational sessions that The Wellness Plan Medical Centers offers are alcohol, tobacco, and drug prevention, and chronic disease management sessions. In addition, we partner with community agencies that specialize in HIV, AIDS, and STI prevention programs, nutritional programs, and physical activities.

Our community outreach team distributes information materials about our services to residents. Their goal is to serve as a liaison between patients and organizations that can provide services to our patients beyond The Wellness Plan Medical Center’s services. Through these efforts, The Wellness Plan Medical Centers will assist in making appointments and gathering information for patients. In addition, The Wellness Plan Medical Centers can aid in the enrollment of community and governmental programs, like Medicaid or other MI Bridges programs.
